Issue: Pixelated hi res print ready PDFs saved out of Illustrator

Question

I've been using the same Illustrator file for years without issues. It contains a linked high-res black-and-white TIFF illustration. After only updating a number, my exported print-ready PDF is now pixelated. The file is clearly hi res at almost 150MB, I double checked everything, re-linked, re-saved, etc.

Other troubleshooting steps taken:

Uninstalled/reinstalled Illustrator & Acrobat
Restarted system & cleaned up files
Resaved the file and ran multiple tests

Still no resolution. Any insights?
