Please help: Is there a way to make interactive fill-out forms in any Adobe Software and export them

Hello

My teacher gave me a task to create an interactive fill-out form using Adobe Products. I am not familiar with this and have no idea if he is just testing my knowledge, or if this is really possible. Here is the task he gave me:

  • Design the form in Illustrator or InDesign and export it only in PDF (this I understand). What I don't understand is how to make it interactive:

  • The form needs to have squares with questions (Who is your favorite writer, What is your favorite band, etc).

  • Once the user clicks on those squares with questions the question disappears, and they can type their answer instead in that same square.

  • Everything in the document needs to fit a specific color palette.

  •
