Swatch Conflict won't go away

I have an Illustrator file with shapes in different colors that are colored from a custom swatch palette. Whenever I select green then the "Swatch Conflict" window pops up saying "The color swatch "C=54 M=11 Y=88 K=0" has a conflict with the current document." It wants me to Merge or Add swatches and there's an "Apply to all" checkbox. I have tried every combination of selecting things and it never stops popping up. The document color mode being CMYK or RGB makes no difference.

In a new file there is no issue using this custom swatch palette. 

How do I fix it so the error message stops popping up? Thank you!

    I figured it out! There were no hidden layers. It was duplicate green swatches that had been accidentally created in the swatch panel that was messing with things. I deleted all of those and the window stopped appearing. Thank you for your help.

    Without any idea about teh actual contents of the file nobody can tell you much, but such issues all too often boil down to color profile problems or "tained" artwork having been imported and placed on the artboard, bringing odd colors in. also double-check your layers palette for potential "garbage", i.e. invisible objects hidden behind others or paths and stray points that may hold color info that gets in the way.

    I figured it out! There were no hidden layers. It was duplicate green swatches that had been accidentally created in the swatch panel that was messing with things. I deleted all of those and the window stopped appearing. Thank you for your help.

    YES thank you that worked! I pasted in a snip of where the swatches you should delete are. Because my saved swatch list "SR Swatches22" ,as pictured in the snip, had copies in this "Swatches" main tab it was giving me the error.
