Why are my shapes creating phantom images behind them?

My boxes are creating phantom images that are only visible at the bottom of the shape. They seem to be words that are either elsewhere on the file, or had been and then deleted. I've ungrouped so many times that there is no way the image is actually there. I've updated Illustrator. I've exported it to see if it was just a display glitch, but it won't go away, even when printed. It seems to go away with a clipping mask, but I'd like to fix the actual problem.

    How do you create those boxes, just draw a rectangle and the phantom shapes appear?
