Why does the text sometimes end up outside the text box?

It is years I'm wondering why sometimes the text (expecially punctuations) ends up outside the text box. Does anyone have a solution to fix it? It doesn't seems to be a problem of optical margin.

OK - It didn't work while the Preview option was checked/enabled, but it worked a few seconds after I clicked OK!
