all brushes are grainy

I don't know what's going on but any brush I use is grainy?? How do I fix this, it's super frustrating. Even with smooth, flow, and opacity at 100% I still get grain, this is the regular brush (flat point). I'm at 100% zoomed in, please help!!

Check the Brush Mode. It should be set to Normal.
