Element not exporting to PDF

Recently, I purchased a Resume template and have used photoshop to edit it with my picture and information.

My photoshop skills are about a 1/10, so I'm not sure why I'm having the following problem. Everything looks fine in the PSD file, but when I try to export it to PDF to upload it so I can send to potential employers, one element is missing from my resume. I have no idea why this would happen.

When I see it in the PSD file it looks like this:

but when I export it to PDF, it looks like this
