How did I do this? (Photo to black / white / gray value "shapes")

I know it can be done because I've attached the evidence. And it can't be TOOOOO complicated because I did it. But I can't for the life of me remember how.  Help? 

 

GOAL: Create an image from a color photograph that lets me create sections of that photo by value. Maybe 4 values, maybe 10 or more. The important thing is that unlike "posterize" the end result has defined "shapes" that I can use as a template for collage, etc.

 

See the attached. The original image, and the outcome that I am trying to replicate!

Looks like one or more Filters from Filter Gallery.

 

Unless you saved it as an Action or wrote down the exact method & settings used, it may take some repeated trial & error to replicate what you did last time.

 

Go to Image > Mode > Grayscale.

Copy Layer with Ctrl + J.

With topmost layer selected, go to Filter > Filter Gallery > Artistic > Cutout. 

Adjust values to suit.

Image->Mode->Indexed Color, set to a low value and turn off dithering.

Is it possible you did it in Illustrator with Image Trace?
