Image squashes when exported on Photoshop

Please help: When exporting an image on photoshop with a 16:9 ratio, it appears squashed and distorted.

I transferred it to a new document but the same thing happened.

With the exception of viewing some very specific video clips, pixel aspect ratio should normally be set to square pixels.

Could you please post screenshots with the pertinent Panels (Toolbar, Layers, Options Bar, …) visible? 

What is the View > Pixel Aspect Ration setting?

Thanks, but I've managed to fix it by copying it onto an art and illustration document instead of a custom one. However, the Pixel Aspect Ratio was...
