Issues with layers

Having a nightmare trying to sort out something hope you can help please! Probably a really simple fix if you know what the issue is. Video attached as it a bit hard to explain, but basically trying to use the patch tool or blemish tool on its own layer and wherever I have used another tool on another layer it wont work in that same area and makes it worse than the original. Can anyone advise what I am doing wrong please? TIA

in the video you can not see what's in the layer groups, therefore it is difficult to judge where the mistake lies. Just an idea: Check the option "use all layers" in the options bar.

do you mean sample all layer? as if so I have tried that too with the same issues

Yes, I mean "sample all layers" (I use the german version)

Then just create a retouch layer on top with cmd/ctrl + alt + shift + E and retouch it, it should work in any case

Without audio  your video is useless there are too many layer and layer groups above your image layer It not possible to follow what you are trying to do.  The layer mask thumbnails are so small so black it looks like the layers above the image may not effect the document composite . After playing the action you are directly editing the covering image copy layer and it is a destructive edit you can lower the layers opacity to recover some of the original image quality.

It seems to be my de-blothc & magic esraser actions that I use that are causing the issue.

the magic eraser is a bought action and this is the contents of that action, does that give anymore clues? Thank you in advance
