Photoshop 22.0.1 Spot healing brush broken

Please see my screencast to see what is happening with the Spot Healing Brush.

Here is what I have tried;

• Reset the brush
• Reset all tools
• Reset Photoshop Preferences
• Relaunched PS
• Rebooted iMac Catalena 

I'll try to uninstall/reinstall PS next.  Any other ideas?
