PhotoShop CS6 is confused!!

I am and have been using Photoshop CS6 (64 bit) for the past few years with no problems. Recently while editing my photos (The process I use follows these 3 steps.  1- Adjust Levels; 2 - Adjust Curves; 3 - Adjust Hue/Saturation) I get a strange problem.

Step 1 (Levels) works fine and causes no issues

Step 2 (Curves) is where the issue occurs. I select "Curves" via the Create new fill or adjustment layer icon located in the lower right of the screen and another Levels adjustment box pops up, but no Curves. Hue/saturation works fine. Once in a while I am able to get the Curves box to show up, but the histogram has vertical lines thru it.

I was going to uninstall and then reinstall Photoshop, but I cannot seem to find my disk

Any help would be greatly appreciated

    You are working in 8 bits per channel which only gives 256 possible levels for each color (RGB). Applying a curves adjustment after a levels adjustment (or vice versa) will start to introduce gaps in fact with radical adjustments the histogram can start to look like a comb.

    The solution is simple - convert the image to 16 bit before applying the adjustment layers.

    Looks like the regular Curves display to me.

    What have you done for trouble-shooting so far?

    Restoring Preferences after making sure all customized presets like Actions, Patterns, Brushes etc. have been saved might be a good starting point:

    Could you please post screenshots the pertinent Panels (Layers, Channels, Properties, Options Bar, …) and dialogs visible?
