Photoshop transform bug

Hi has anyone found with the latest photoshop update that when you transform an object it does transform but also still leaves the original within the  same layer?

Any ideas how to fix this?  I've managed to get rid of it by opening and closing the file but then I can't be doing this every time I need to transform.
