PS tabs showing same image

I'm having a recurring issue where if i have a few tabs open in PS, after a while they all display the same image - usually the most recent one - and all it's layers. Even though each tab has a different file/different image, PS keeps showing them as being all the same. I then have to quit PS and start again. Anyone else experienced this?
