Rectangles appearing for no reason

I was editing 4 photos today for a client (apologies for the blurry model, but these photos are not released to the client yet) and got a bunch of strange rectangles appearing in what should have been a uniform background. I'm no stranger to editing and did 4 photos exactly the same. At no time did I use an area selection tool (other than select subject) and I have not added anything at all in those rectangle areas.
Weirdly, the first time I noticed it was when I was just about to save it (as a finished image), and it appeared as the one line you can see - dividing the background into two colours. It only appeared in some zoom states, and not others, so I assumed it wasn't really there. When I saved and reopened it and PS, it was there in all zoom views. I could also see it in LR Classic. I restarted my iMac ang again it was still there. The one with even more blocks was one layer down in my editing process and while not visible, I have no idea where they all came from. As it was the very end of the editing process I had to just edit the background back to uniform again, so no ongoing issues. However, any idea what this was, and why it only happened in one image? I'm no PS expert, but have been a full time photographer for 13 years (prior to covid) and have edited thousands of photos. First time I've ever seen anything like this. I'm pretty positive I've not done it inadvertently.
