Refine Edge issue

I'm having an issue with the refine edge brush or Smart Radius tool.  It leaves light grey areas in hair when applied.

They are not visible when composited over light backgrounds, but they are using darker backgrounds.

I've tried adjusting every single setting all with the same result.

I'm using v. 2015.1.2 with an Invidia GTX 570 graphics card.

    As a starting point it's not too bad.

    Add a new layer.

    Make a clipping group.

    Paint or clone on that layer.

    For high quality results on images that are ill-fit for the task the feature may just not suffice and one may have to manually decontaminate certain areas (and possibly also have to improve the Mask).
