Saving file still shows asterisk and won't save until I hit cancel and try again

Sometimes saving a file just won't happen, where I ctrl-s a file after updates and it keeps coming back asking to save... to a point where I have to click cancel and try again. Or I click save, it shows it saved the file but there still exists the asterisk showing no.
