Trouble with refine mask in Photoshop CC.

I select my image and hit refine mask I go along the edges. I am using the refine edge brush tool on the left I finish and when I go to output, I select layer mask. 

 

The outcome shows  a blurry, foggy, smeared look where the brush went and I can not figure out why.

 

I am includin screen shots of my settings and results

Hi David

Try resetting the Select and Mask workspace using the Workspace Switcher in the upper right. What happens when you change the radius from 2px to 0px?

Dear David,

I'm having the same issues since few weeks (before that it worked perfectly), I was looking for someone talking about it...

It's not only me but all of my students that should work on a composit assignment and they can't.

Please somebody can help us?
