Can anyone help me with how to take an image and make it look like this for a tshirt?

The face is not vector; it is raster. Photoshop does not convert to vector, but Illustrator does. The results with this particular image might not work well , but you could try.

 

For the text, use the Type tool.

 

If you need a vector format, use Illustrator or possibly pdf.
