Issue: M4V and ADTS instead of one MP4

Hello, every time I export a video from Adobe Premiere Rush in mp4 format (and save it to local destination/externald hard drive), it exports two files instead: one m4v (which contains the video) and one ADTS (which contains the audio).  How can I export one video which contains both the video and the audio?

My export settings are:

Preset: Automatic

Resolution: 1080p HD

Frame Rate: Automatic (29.97)

Audio Channels: Stereo

Quality: High
