It looks like you're not doing anything wrong.

These governement forms are usually outdated and were not created with Adobe Acrobat Pro DC.

This simply means that it is possible that the creator who produced this PDF didn't consider adding Reader-enabled rights.

Assuming that this would be true, you won't be able to certify this type of document using the Certificate tool.

The form already provides (or should already have) a digital signature field.

If you are using Adobe Acrobat Reader to fill and sign that form, you won't be able to sign with a certificate-based digital signature, and neither will you be able to employ electronic signatures via the "Fill & Sign" tool due to the security restrictions that were set in place when  that PDF was produced.

You can try using Adobe Acrobat Pro DC instead.

Adobe Acrobat Pro DC may allow you to click on the provided signature field and apply your certificate-based digital signature.

I'd suggest to test with a blank copy and not one that is already filled out with sensitive data.

That said, if this is happening also with Adobe Acrobat Pro DC, then make sure that you are connected to the Internet at signing time.

Most of these government forms also need to validate your certificate's file credentials (such as expiration date and the SHA  hashing algorithm for proper encryption/decryption),  provided that a root certificate authority is trusted and a time-stamp server is accessible  online at signing time.

Would you be able to share a screenshot or the name of that government form?

Also state where did you downloaded it.
