Acrobat application closes when trying to prepare a form

Question

I am trying to create a form, using an existing PDF file. Every time I activate the Prepare Form tool, the application closes. I also tried the File / Create / Create Form menu and the application also closed. Is there a way to fix that problem in the app?

I would do the following, some or all of which you may have already tried.

Restart your computer.
Perform a Repair Installation (under Acrobat's Help menu).
Try using a different file - might your original file be corrupt? Does this happen with ALL PDFs you try, or just one in particular?
