Acrobat DC, Prepare Form, Text Fields disappear from pdf downloads

Hi,

I've been creating fillable text form in Acrobat DC. However, the fillable boxes disappear after I download them to my computer and open them again. and open the file as a pdf. the text boxes aren't there, though they are still  in DC.  Here's a two-minute video to show what I'm doing so you can see my process exactly and flag where I'm making the mistake.
