If you want to learn how to design "Acrobat friendly" forms, check out episodes 52 and 53 of this podcast. They focus on InDesign, but the design guidelines are the same:

Hi there

Hope you are doing well and sorry to hear that.

Would you mind sharing more details about the issue you are experiencing?

  • What is the workflow/steps you are doing to prepare PDF form? A small video recording of the same would be very helpful.
  • What is the version of the Adobe Acrobat DC  and the OS you are using? To check the version go to Help > About Acrobat and make sure you have the recent version 21.11.20039 installed. Go to Help > Check for updates and reboot the computer.

Also please go through the help pages listed below and see if that helps:

I work from Word.

Create a table with bottom lines where imput is needed

import this in Adobe Acrobate Pro DC

Select prepare form

What works:

  • Ballot symbols are autmented with Adobe Acrobat DC checkboxes

What does not work

  • Lines are not converted to input fields
