Acrobat Reader crashes on Prepare Form

Help!  I have been working on a form fill document for a month now.  All of a sudden, I open the document (and it still opens), but when I hit "Prepare Form" it just closes.  I've updated to the latest version, repaired it, saved it and opened the saved doc, rebooted several times, tried editing first and then hitting prepare form, but it crashes every single time.  No error message, it just goes away and closes the program.  I need to work on this document and update the form!!!  Please help!

Thanks for reporting the issue. Could you please confirm following:

1. Is it happening with Adobe Acrobat or Adobe Reader or both?

2. Please share the exact steps or a short video if possible.

3. Check if the workflow is working with Protected mode OFF in Sandbox Protections (Open Reader/Acrobat > Press 'Ctrl + K' > Navigate to 'Security (Enhanced)')

4. Please share the logs/dumps using https://helpx.adobe.com/in/acrobat/kb/collect-logs-crash-freeze.html

There's no Prepare Form command in Reader. This is an Acrobat issue. I'm moving it there.
