Acrobat XI re-install

Hello.  I was referred here by an Adobe chat agent who confirmed that I have an active perpetual account - I presume it is for Acrobat XI.  A Windows reset uninstalled my Acrobat XI but I still have the folder with the set-up exe and I started it but was stumped when asked for username, organization, serial number.  I found the serial number in my Adobe account but still don't know what I might have entered for username and organization.  Please advise.  Thanks.
