Action button in creating fillable forms

I am having trouble creating a "Clear Form" button and "Submit" button in Prepare PDF because the "button properties" box does not have an "action" option.  Does anyone know how to add this action option?

I do not see "Prepare PDF" in the "Tools" provided in Acrobat. There is a "Prepare Form" tool.

How did you create your PDF and exactly what tool are you using to edit the form?

If you used In-Design and added the fields within In-Design, delete the fields and start over using the "Prepare Form" tool in Acrobat.
