Add/remove words under highlighted block...

Is there an easy way of lengthening or shortening the width of the higlight over text, without loosing associated comments?

For example, if I've decided I should have highlighted the entire sentence instead of just a phrase.

I've tried various mouse gestures but haven't yet found the right ones.

No, not really. Just copy the comment, delete the highlight, then create a new one and paste the comment into it.
