Adding and Assigning Additional e-Signature Fields to a Form

I designed a pdf form that my employer uses to collect the signature of an employee who is to receive a company credit card signifying they agree to comply with the credit card policy.

It requires e-signature by themselves, their line manager and the Head of Finance.

The form in its unsent/ template state has the 3 e-signature fields that are assigned to each signer.

I usually just open the form, add the recipients via the ‘Request e-signatures’ option and send it on.

We have used it for 3 years, but recently, an employee who signed the form commented that, although we have date specified on the form, there is nowhere to input it without printing and hand dating it, which defeats the object of the exercise.

I have added a date field underneath each of the 3 e-signature fields and formatted them accordingly.

 

When I open the Request e-signatures option, all 3 date fields are assigned to the first signee

How can I now assign each of the new date fields to each recipient?

Online documentation seems to be pretty vague to me and rather unhelpful.

Do I have to wait until I need to send the form out again, change the recipient in the Line Manager and Head of Finance date fields and then save once its been sent so that it assigns them for future use or is there a way to do it without sending it for e-signature first?

 

Apologies if that sounds vague.

    How do I save the document as an Acrobat Sign Template?

     

    When I click Save As, I only get the options to save as:

    Adobe PDF Files (*PDF) & Adobe PDF Files, Optimised (*.pdf)

    When I click Save As Other, the options I get are:

    Reduced Size PDF, Certified PDF, Reader Enabled PDF (with 2 sub options: Enable Commenting & Measuring & Enable More Tools), Optimised PDF, Archivable PDF, Press- Ready PDF & PDF/E

    I am using Adobe Acrobat Continuous Release, Version 2026.002.21931 64 bit (in case that has any relevance)

    You can create or save a PDF form as an Acrobat Sign template using Acrobat on the web. Please refer to the following help document for more information: Create agreement templates. You will also be able to access these templates from the desktop application.     Thank you for reaching out.

    It appears that you have added the date field, but have not assigned it to each recipient. Simply placing the date field below the signature field will not automatically assign it to the respective signers. You will need to assign the date field to each signer individually.

    If you have saved the form as an Acrobat Sign template, you can open and make changes. Please refer to the screenshot below to see the option that will help you change the recipients:
