Adding Pages to An Adobe Form

I created an Adobe form that was working great, knowing I was waiting on our newest operational policies. Now, when I add the pages for the new policies, my fields have all disappeared! Please help!

Hope you are doing well. Sorry for the trouble with working with forms.

 

Is this issue you mentioned, seen after combining the files or on viewing the initial file? 

Would you mind sharing the files with us to test in-house?

 

Also, the Acrobat version number you are on (go to Menu-> Help-> About Acrobat Pro DC), and the OS details would be great for further investigation.

 

Please ensure you are on the latest version of Acrobat for the best experience.

Thank you so much! I've made a slight improvement since my post. I can now "see" fields, but I can't do anything to them. When I click "Prepare a form" it autofills the fields with new field boxes, but I can't remove the old ones.  I'll attach screen shots, including my version number.

The "treehouse" form you've posted is filled, flattened, and signed. It looks like it has been through the Adobe Sign process. It is not a working form with fields.  

Can you post the actual form(s)? Before and after adding pages.  Did you mean to create an Adobe Sign form?
