Adobe Acobat 9 Pro Extended - Mailmerge plugin to Forms

Help please!

I've spent quite a 'few' hours investigating how to create multiple individual forms in one 'go' from a spreadsheet. It looks that I need to use a plugin called 'Mailmerge'. This is where I'm struggling, I just can't find it and can't see where I can get it - if someone could point me in the right direction it would be appreciated.
