adobe acrobat reader DC... check boxes selects all ? instead of just the selected box

Does anyone know how to correct the following... when I'm working within Adobe Acrobat Reader DC, I select a check box for my client applications, but instead of the check box that I'm selecting getting highlighted it selects all boxes... is there a way to fix this?  so that it only selects that box that I want selected?   Please advise any help is greatly appreciated!  🙂   Thanks

Adding on to the expert's response.

Adobe Reader doesn't play any role when it comes to form fields and its properties, because its freeware app and it just help you viewing your PDFs and annotating them. 

How your form's check box and drop box works that's completely depends on what settings/value applied to form's fields. You need to check with the developer, as it cann't be changed from your end.

I agree with gkaiseril that all the checkboxes have the same name. To fix the problem, give each one a unique name.

It sounds like all the check boxes have been given the same group name and export value. Contact the developer of distributor of the form and ask them to correct the problem.
