Adobe eSign Creating Duplicate Signature Field

I am creating fillable PDFs forms. I am using text tags in Acrobat Pro DC and have my eSign fields on. All of my other fields populate and merge as I envision from my webform (client fills out web form - data merges into PDF - they sign document via Adobe eSign - boom they are done and sent a copy of document.)

 

I have room for up to 5 signers on the PDF. I have added, deleted, and readded the signature and date fields and I cannot figure out why Adobe eSign insists on readding a new page with the signature and email address of the signer at the top of the new page.

 

We do not have company settings requiring company and title to be added - I have done this successfully on a different PDF form and had no issues. I have looked at the settings on that form and can not determine the differences between the two. Any guidance to what I am missing would be greatly appreciated! Thanks so much!

I am still having this issue. I have looked through all the other similar posts and replied and posted and not received a response. I have looked through all help documents and references and resources and am at my wits end with this issue. Can someone from Adobe please help me with this? We are using the API filling the form out from a web form before it gets signed. But again, I have done this with another form successfully with no issues. I need this to be done repeatedly and efficiently in the future. A response would be appreciated.
