Alt text for tables is not read on PDF

When I Converted word to pdf the alt text for tables were given already. I see it on the table properties as well but the screen reader is not reading it out. Also under accessibility the set alt text shows no figures. Please suggest.

I've tried the latest versions of word, acrobat and the following browers and screen readers

Edge, Chrome, Windows Narrator and NVDA

Tagged heading levels are not being read by screen reader. H1 is being read as Heading 2 and the heading 2 is not being read out. Please suggest.

The bullets have been tagged as a list on the pdf file but it reads all of them as one sentence. Please suggest.
