Alt-text getting cut-off?

I just wanted to check with you guys if any of you encountered a scenario that alt-text of selected images got cut-off after the file have been save and accessibility check has been made. This is the first time I've encountered this said scenario. I am using Adobe Acrobat Professional XI. Any help would be greatly appreciated. Thanks.
