​@Jacyara Chances are you not converting the file. If you save the file as a standard PDF, some settings might strip the profile. Saving as PDF/X-4 is generally better for preserving color intent. And in my set-up if color isn't an issue, I change the setting to "Discard Color Profiles" in the Save settings to reduce file size.
 

The best way to convert it is through the Preflight tool under the Print Production tool. Click on the Select Profiles tab (the wrench icon is for "Fixups," which is what you want). Search for "Convert" or do what I did, enter profile in the search field to speed the process up!  
 

Click the Edit button (or double-click) to select the specific sublimation profile from your system. Choose your color profile. Click the Analyze and Fix button at the bottom right. Acrobat will ask you to save a new version of the PDF.
