Can you create a signature block template?

I am using Adobe Acrobat Pro to get invoice approval prior to payment.  I want to create a signature block that I can use over and over again without having to change it.  Is it possible to create a signature block that would have the following fields automatically set up that I can assign to each bill approver with one click:

 

Approved by: (I currently manually pick Signature under Signature fields)

Date(I currently manually enter the Date from the Signer Info Fields section)

Comments: Signer enters any comments about the bill (don't pay until xxxxx, project complete, etc).  I currently manually enter "Text Input" field under data fields, make sure it is not marked as required, and have to manually  change the font from auto to 10 under Appearance, or the comments are crazy huge font.

 

I can't find anything about creating a custom block, and I've done this repetitive, annoying task way too many times in the last 6 months.  Either it's not available, or I'm not searching for the right term in support.  Can anyone point me in the right direction?

You can use a Stamp to add the template, but the fields in it have to be filled in at the moment the stamp is applied, and then they are flattened, so if you want to use a Signature field that would require a more complex solution.

Another option is to add the fields directly to the page, using a script, but that would depend on their location always being the same. If it's not you'll need to manually move them to the desired location after they were applied, or mark that location in advance, for example using a comment such as a sticky note.

Either way, it requires the development of a custom-made script.
