Check boxes issue- all boxes being checked automatically

I created a bunch of checkboxes where people can select 1 or as many as they want. But, when I tested, I check mark one box and all the boxes get the check marked. How do I make my form that people can check multiple boxes individually? I named each box differently but it's still checks all boxes at once. Pleeeease help! Thank you
