Check Boxes not saving when recipient recieves an e-sign request

Hello All,

 

Currently having an issue where we're pre-filling some fields of a form which has check boxes and text fields, but when requesting an e-signature and the recipient recieves the e-sign and goes to complete it, the check boxes that were previously checked are now empty, but the text fields still have the info before sending them. I've seen fixes of saving the semi pre-filled file seperately, then requesting with that file, but I'm looking for an easier fix for users not having to do that. Thanks.

Figured out my issue. Something with Adobe's auto create system where it auto creates text fields and check boxes based on the file makes the request signature wonky and unchecks those boxes when the recipient recieves them. After manually recreating the checkboxes with the tool in Prepare a Form, it now works as intended.

I’m having a similar issue, except everything shows on the preview and even for the recipient it shows the checkboxes and radio buttons selected, but once signed, it deletes all of those selections. Doesn’t matter if I do them prior to sending or the recipient does them during signing. They never come through.

Please let us know if you prefill the form in Acrobat or using another application. How are the form fields added to the PDF form?

Could you please share the sample prefilled PDF in which the issue occurs? It will help us to replicate this behavior and share the correct steps to fix this issue.

The form isn't prefilled originally, we fill out the form partially when getting a request since there's alot of options and they change so we partially fill it out and use request e-sign to send it out. I used Adobe with the Prepare a Form function to create the file. Here's the file that I've been using if you want to try yourself.

When you send the form using e-signature, do you see the checkboxes filled in the preview form. Also, when you create a form, ensure you have 

"Prepare form for e-signing selected, that way the fields are compatible with e-signature services.
