Checkboxes only recognised on one page of PDF when converting to a form.

I have created a 3 page document in Word which had checkboxes on all three pages, which I then saved as a pdf.  When i open it in Acrobat, and try to change it to a form, it only picks up the checkboxes on one of the pages, but not the others.  I cannot work out why this is happening as the checkboxes were all created the same way in word. I have to then manually create and size the checkboxes on the other two pages.  Any suggestions please??

Are you doing it using the Fields Detection Wizard? If so, it's hard to say for sure why it works for some fields and not others. Could be the context in which they are found, such as text or other graphic images.

If you want 100% accuracy then you need to use a script to detect and create your fields, such as these (paid-for) tools I've created:
