Checklists Data Dissapears

We use a checklist created by AICPA that asks due diligence questions on tax returns. We fill out the checkmark yes/no questions, save and close. When the file is opened on another computer or even the same computer sometimes the yes/no questions are blank again. It is never the same questions, sometimes it is all of them or just a handful. Can you please help me with why this is happening and how to fix it?
