Combined pdf error

When I combine multiple pdfs, the combined document contains multiple versions of one of the documents and is checking off check-boxes that were not checked in the original documents.  Please help.

If the files you're combining have fields with the same name in them they will become a single field (or field group) in the combined file, with the value of the field from the original (or first) file.

Thanks.  I am not sure how that happens as not all the check-boxes change, just some of them.  The greater concern is why one of the pages is being replicated three more times instead of the three that should be there.
