Combinging a PDF'd Word document and a fillable form causes scrambled text.

As you have mentioned, the header and footer text in the PDF get scrammbled when you combine a Word file and a form. Is that correct?

Did you check if the issue occurs when combining these two particular files? Try combining a different word file with form and check if the issue still occurs.

What is the applicaiton and OS version installed on your machine? To check the applicaaiton version, use these steps: https://helpx.adobe.com/acrobat/kb/identify-product-version.html

Could you please share the screenshots of the PDF created after combining the files?

I should have mentioned the scrambled text appears on the PDF'd Word document not the form when they are being combined.
