Combo box (drop down list) not working

Hello Guys,

I have gone through most of the post and just couldn't find my error. 

I am trying to build a multi-filed drop down on my form and used combo box feature. However, I keep getting an error that a syxtax is missing, but i can't actually find the missing syntax. Have tried all I could.

Can anyone help me out on this? I don't seem to know its wrong.
