Composing multiple choice with options listed horizontally

Hello, I am composing a fillable form and am hitting a roadblock. In the form, there are nine options under one of the headers and design wise, it makes much more sense to have them three down and three across on the form. The issue is, the person filling out the form can only pick on choice. Is there any way to link the options both vertically and horizontally? It can defaultly link vertically but I am unable to add to that menu. Any advice would be appreciated!

P.S. I know worst case I can specify to only pick one but for the sake of concision and future reference I wanted to ask!
