Crash when creating form

I am attempting to create a form from an existing pdf. With one particular PDF I get errors when I either:

- Use the Auto-detect form field
- Attempt to manually input the fields

When attempting to use the auto-detect Acrobat freezes, goes into a 'Not Responding' state and must be force closed. This same behavior occurs when I add a single form box manually.

This only occurs with one PDF. I have done several others. All of these PDFs were created and exported from InDesign. There is nothing fancy in them. Its text, boxes and tables.
Attached is a diagnostic. I am not sure how helpful it will be, as Acrobat will hang indefinitely while the diagnostics is running and will crash immediatly after diagnostics are stopped.

I'd start by looking at any linked files you may have added to the InDesign version of your document, then I might try exporting the document from InDesign as a print (as in, not an interactive) PDF, then open the document in Adobe Acrobat and use the File>Save As menu command to rename and save a copy again before trying to open/place form fields.

 

These functions help create a simplified PDF to start from. Testing without the linked files ensures that you're not dragging something along placed the InDesign document as the root of your problems. The double-saving process helps streamline the file itself. Between the two, you can hopefully find the root of your problem and correct as necessary.

In addition to the good information above, check for and install any Acrobat & InDesign updates, if you're still having issues, run two preflight checks, first for Online publishing, if the preflight report shows nothing wrong, run an additional preflight for PDF/X-1a, which might flag more issues. (Tools> Print Production> Preflight)
