Creat PDF form that merges a locally stored PDF with personal data

Hi .. I have mainly Reader experience with Acrobat.  We have activity based permission forms sent regularly to parents which they then complete, sign & email return.  The problem for parents is that each form contains personal data that must be completed for each new activity that needs to be signed. Is there a way for Acrobat to setup a form structure where;

1. Each parent can complete a personal data PDF form once for their child and then store this on their computer for later use

2. Each time they receive a new activity form .. they can attach the personal data form by clicking a button

The soliton needs to be very simple, intuitive and accessible via Reader as we can't require parent s to purchase Acrobat.  Can this be done ?

Indeed social media is worse but email is surely unacceptable. Use https. In Europe there are unlimited fines for this Sort of thing.

Yes that is a better solution but the organisation does not have the expertise, resource or focus to set this up themselves.  They use PDF's now and that works .. I'm just wondering if there is way to improve the current process.

Surely asking parents to email personal info is a huge breach of confidentiality and would be illegal in all of the EU at least?

I agree that is an issue .. but this is probably better than sharing on a social media platform where the privacy statement means very little.  Also a  PDF can be printed and handed in which is also fine.
