Create a Dymo Label from info from a PDF

Question

Hello all I was wondering if there is a way to create a dymo label and have it auto fill fields in the label. Right now I have a form that we use here and i also have a label that needs to be created using the same information that is in the form. Or can a something like a script be added to the form so that the label is one of the pages and it tells it when printed the page the label is on goes to the label printer instead of the Epson.
