Create Intake Form for Client - signature and photo ID

Question

I would like to create an intake form for my clients that requires them to sign and upload a photo of their ID.. What would be the simplest way to achieve this?  Similar to what I have used at the doctor's office.  Ideally, there would be a button to press on the form labelled "upload photo ID", that would open the camera on the iPad, allowing them to take the photo and then automatically add it to the form.  Similar procedure for the signature, except it would open a signature field where they can sign and save.

Please refer to the information provided about the new feature "In-line Images" in Acrobat Sign: In-line Images. will help users to add image to the form. Check if you can use it as your required.

If not, more information would be helpful about the workflow. If you can share any screenrecording of the form you are referring will help us understand it better.
