Created a form with conditional checkboxes and when I request e-signature, they don't work

Please Help! I created a form with conditional checkboxes and text fields that fill a specific number when one of the checkboxes is checked. But when I request and e-signature, those don't work. They work when i do the preview, but not when sent for a signature.

 

When I click Month-to-Month check box, it checks the Month-to-Month box in the next box to the right. When I click 12 Month (or 24, or 36), it checks the box in the next space that says...I want to lock in my rate and populates the text fields to say 12.  This works when I do it in Acrobat, but when I send it for a signature, none of those work.

Because the file is converted to an Adobe Sign form, which has limited functionality.

Instead of sending it for a signature, just add a Signature field to your file and a submit button to send it back to you, and email it directly to the recipient(s).

Will the signer have to have Adobe Acrobat to fill and sign the form? I just want it to be super easy for the customer. And if I do it the way you are suggesting, is it still a legal document?

 

One other problem I'm having...before I send the document, I pre-fill some of the info like the customer name, address, etc. and some of the features they have already chosen. But when I send it a lot of that info gets erased. I read on the forums that is a common issue, and to resolve it, the only thing that has been working for people is to revert to the old version of acrobat. Is there a better resolution?
